free books for NC nursing home residents
In 2020, 2nomi was awarded a Civil Monetary Penalty grant by the Centers for Medicare and Medicaid Services (CMS), administered by the North Carolina Department of Health & Human Services (NCDHHS) and the North Carolina Culture Change Coalition (NCCCC) to enhance person-centered care practices at 60 North Carolina nursing homes. 2nomi Integrates innovative, evidence-based person-centered care research into clinical practice by developing and delivering products, training, and services for healthcare providers.
The three-year project is titled ““To Know Me – A Method to Enhance Person-Centered Care”, a program that will benefit nursing home residents and staff. 2nomi helps the resident and their family celebrate the life story of their loved one by creating a 2nomi life story book to share with staff so they may develop relationships and knowledge that improves person-centered care (PCC). 2nomi then provides staff training on PCC principles, integrating life story into care plans and CNA care guides, use in activities programming and with residents exhibiting expressions of unmet needs.
Life story books help nursing homes develop comprehensive care plans and assessments based on a resident’s life history. The books are an effective tool to enhance care by promoting a culture of person-centered care that may lessen anxiety and increase residents’ quality of life.
Residents and their families receive instructions and support to create a beautiful photobook that tells the life story of their loved one.
Facility staff receive on-site training on how to use the books in clinical practice. Outcome indicators will be assessments of staff person-centered care practices and a satisfaction survey for all participants. By the end of the 3-year project, 6000 NC nursing home residents will have received a deluxe life story book to share with staff of 60 nursing homes that have integrated the life story books into their person-centered practices.
